On Your 11th Birthday...
 
All students need to receive their Tdap and Meningococcal immunizations on their 11th birthday! When your student turns 11 please take him/her to receive these from your doctor. 
Per AZ state law, 6th grade students who have turned 11 yrs. old will not be allowed to attend school without proof of these vaccines.  When immunizations are complete please return proof to our
Health Office.
 
If you do not have a doctor or cannot afford them the Chandler CARE Center can help. Their information is provided below. 
You can also visit the Health Office for additional help.
