New Transcript Request Procedures effective January 23, 2013
Current Students - To expidite our transcript process, CUSD has partnered with a third party transcript vendor, Parchment. Transcript request forms are no longer needed and there is no longer a two week processing time. To request your transcript you will first create an on-line account with Parchment. Once you have created your account, you will be able to request to have your official or un-offiical transcript sent electronically to the institution of your choice and/or to yourself. Instructions for setting up your account are included in this attachment.

Parchment will also send your transcript to NCAA and the Common Application.

Need Help? Once you have signed into your student portal, "Parchment Student Support " will be happy to answer any questions you may have regarding the new transcript request system.
2011 Graduates and beyond will need to contact the CUSD District Website at this link to request a transcript.
2012 Graduates
will use the same form in the above link and, return it to Perry High School Registrar's office for processing.
